Primary Total Hip Arthroplasty with Impacted Morselized Bone Graft for Old Unreduced Acetabular Fracture in the Young Patient

Abstract
Treatment of the displaced acetabular fracture in the young patients is usually the open reduction and internal fixation. But the neglected and unreduced acetabular fracture could be treated with the primary total hip arthroplasty. The indications for primary total hip arthroplasty may include marked impaction or extensive abrasion of the femoral head, marginal or central impaction of the acetabulum involving more than 30 percents of its surface area and extensive acetabular comminution in the presence of osteopenic bone. However, these are the cases of elderly patients and there are a few reports of the primary total hip arthroplasty as a management of the displaced acetabular fracture in the young patients in the foreign literature but authors are not able to find any reports in the Korean literature. We experienced a 32 year-old-male who had the neglected acetabular fracture with central dislocation of the femoral head. The patient was treated with cementless total hip arthroplasty in combination with impacted morselized bone grafting of the acetabular defect for stable cup fixation. We report this case at 4 years follow-up.
